Job Description

FUNCTION:

The Buffalo Promise Neighborhood Case Manager will support all aspects of the Two Generation Program.  The Case Manager will work in tandem with BPN Director of Two Generation Programs. The Case Manager will provide support to the Two-Generation Program that is funded by the Annie E. Casey Foundation to enhance the quality of client/family services. PAZ programs include services at Westminster Community Charter School, Highgate Heights Elementary, Children’s Academy and Gerard Place.

POSITION RESPONSIBILITIES:

The Case Manager will collaborate with families, partners and school administrators to improve health and academic outcomes of children enrolled in BPN programming to include our partner agencies at SUNY Erie Community College, Belmont Housing Resources and at BPN’s four schools; BPN Children’s Academy, BPN Children’s Academy at Gerard Place, Westminster Community Charter School, and Highgate Heights Elementary.

 

PROGRAM RESPONSIBILITIES

Collaborate with our BPN partners (SUNY Erie Community College, Belmont Housing Resources) at BPN schools (Children’s Academy, Children’s Academy at Gerard Place, Westminster Community Charter School, and Highgate Heights Elementary).

Provide direct supervision to the Parent-As-Teacher Specialist

Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered to BPN families

Provide case management services to families, provide linkage and referral, home visits when appropriate

Help clients achieve wellness and autonomy by assisting them in making informed decisions while acting as their advocate and support

Develop effective working relations with BPN partners throughout the case management process

Record case information, accurately including all necessary forms and collected data

Assist Director of Two-Generation Programs with any program issues and concerns

Provide additional support to assess parents’ overall mental health and coping abilities using ACES; connect them with the resources they need to successfully manage their situations/ conditions on their own

Help parents access the wide variety of programs available to support job retention and stable housing, connecting parents to programs and/or the benefits, services and/or supports (could include direct assistance for SSD, HEAP, SNAP, etc.

Understand and adhere to the Company’s risk and regulatory standards, policies and controls in accordance with the Company’s Risk Appetite.  Identify risk-related issues needing escalation to management.

Promote an environment that supports diversity and reflects the M&T Bank brand.

Maintain M&T internal control standards, including timely implementation of internal and external audit points together with any issues raised by external regulators as applicable.

MANAGERIAL/SUPERVISORY RESPONSIBILITY:

Direct supervision of at least one direct report

 

MINIMUM QUALIFICATIONS REQUIRED:

7 years of combined education and experience in the field of case management

Proven working experience in case management, including as a social worker, mental health care manager or a related human services job

Excellent knowledge of case management principles and program management

Previous experience with mental health, trauma informed care and other aspects of care

Effective communication skills

Excellent organizational and time management skills

Familiarity with professional and technical emerging knowledge

Problem solving skills and ability to multi-task

Familiarity with data collection and outcomes

 

IDEAL QUALIFICATIONS PREFERRED:

Master’s degree in Social work or related field

LCSW or LMSW License

 

OTHER REQUIREMENTS:

A valid license to operate motor vehicles, and have your own means of transportation

Flexibility in terms of the working conditions, which might change as per the situation; to include evenings and weekend

Ability to handle stress and pressure that will be accompanied along with the functions and activities that the job demands

A confident personality and the ability to build rapport among the clients and the service providers
